Sub-Acute Care & Rehabilitation

Faircape Health Institute provides specialised sub-acute care and rehabilitation in a hospital environment for patients recovering after surgery, illness, injury, or an intensive care stay. Our interdisciplinary team of medical professionals, therapists, and nursing staff work together to support recovery, restore independence, and improve quality of life through personalised rehabilitation programmes delivered in a safe, therapeutic environment. 

Acute Care

Faircape Health Institute provides specialised acute care services for patients requiring active, short-term medical treatment following surgery, serious illness, injury, or complex medical events. Delivered within a professional clinical environment, our expert healthcare teams focus on stabilisation, treatment, and early recovery to support the best possible patient outcomes.

Specialised Services at Tokai Estate

Faircape Health Institute Tokai Estate is a leading sub-acute rehabilitation facility providing specialised recovery programmes for patients following surgery, stroke, orthopaedic injuries, cardiac events, cancer treatment, amputations, and complex medical conditions, delivered through expert interdisciplinary care and rehabilitation services.

Amputation Rehabilitation

We provide comprehensive rehabilitation following amputation, supporting recovery from surgery through to prosthetic integration integration and long-term independence. 

Cardiac Rehabilitation

We deliver structured rehabilitation for patients with acute and chronic cardiac conditions, supporting safe recovery and long-term cardiovascular health.

Neurological Rehabilitation

Our team manages stroke, traumatic brain injury, spinal cord injury, Parkinson’s disease, and other neurological conditions across acute, sub-acute, and complex rehabilitation phases.

Oncology Rehabilitation

We support oncology patients across the care continuum, from active treatment to recovery and palliative phases.

Orthopaedic Rehabilitation

We provide rehabilitation following traumatic orthopaedic injuries, joint replacement surgery, spinal surgery, and complex musculoskeletal conditions

Palliative Rehabilitation

We provide interdisciplinary palliative rehabilitation focused on comfort, dignity, symptom control, and family support.

General Surgical Rehab

We manage rehabilitation following general surgical procedures, supporting recovery from abdominal, soft tissue, and complex post-operative conditions. 

Medical Rehabilitation

We provide interdisciplinary rehabilitation for patients recovering from acute and chronic medical illness, prolonged hospitalisation, and medical deconditioning.

Sub-Acute and Acute Admissions Procedure

All Patients - Medical Aid & Private

1. The referring doctor completes the Sub-acute/Acute Authorisation and Patient

Referral Form 48–72 hours before discharge and sends it to casemanager@faircape.co.za.

2. Patient Assessment

Patient assessment conducted by our assessment nurse to confirm clinical needs and admission suitability.

3. Confirmation of funding pathway

  • For medical aid patients, the case manager submits the referral to the medical aid for authorisation.
  • For private patients, Faircape Health issues a quotation to the patient or next of kin for acceptance.
  • Once authorisation, approved funding, or quotation acceptance has been received, the assessment sister will liaise with the treating doctor and hospital to confirm the discharge and transfer date.

4. Transport arrangements are finalised

  • Faircape Health Institute books the ambulance using external service providers.
  • Transport costs are subject to the patient’s funding structure (medical aid rules or private account).

5. Transferring the Patient

The patient is transferred and admitted to the Faircape Health Institute facility on the confirmed date
