Sub-acute care at Faircape Health facilitates the transition and preparation of patients to reintegrate back into their home environments. Once a person has stabilised following an acute incident, but still needs medical and/or physical rehabilitation, they are transferred by their treating medical specialists to sub-acute hospitals such as Faircape Health’s world-class facilities.
Sub-acute care is administered to individuals suffering a broad array of diseases, injuries, or conditions. As such, it requires a wide set of clinical skills. At Faircape Health, we have segmented our services into care divisions of excellence to ensure our clinicians are highly skilled and experienced with every condition we treat. Learn more about our sub-acute care services below.
We provide comprehensive rehabilitation following amputation, supporting recovery from surgery through to prosthetic integration integration and long-term independence.
We deliver structured rehabilitation for patients with acute and chronic cardiac conditions, supporting safe recovery and long-term cardiovascular health.
Chronic Pain Rehabilitation is a medical, physical and psychological rehabilitation program for individuals with chronic pain. It includes a strong exercise focus, helping patients gain pain relief, strength, endurance and flexibility.
Geriatric rehabilitation aims to restore function, enhance residual functional capability, restore lost function where possible, and improve the quality of life, especially for those who are frail.
Musculoskeletal rehabilitation is designed to help the person return to the highest level of function and independence possible, while improving the overall quality of life — physically, emotionally, and socially.
Our team manages stroke, traumatic brain injury, spinal cord injury, Parkinson’s disease, and other neurological conditions across acute, sub-acute, and complex rehabilitation phases.
We support oncology patients across the care continuum, from active treatment to recovery and palliative phases.
We provide rehabilitation following traumatic orthopaedic injuries, joint replacement surgery, spinal surgery, and complex musculoskeletal conditions
Prehabilitation is a unique form of physical therapy which improves post-operative outcomes by improving pre-operative performance. This evidence-based approach to surgery recovery has shown to reduce the length of hospital stay, post-operative pain, as well as complications after surgery.
Pulmonary rehabilitation is a far-reaching treatment incorporating exercise, education and behavioural intervention to help those with chronic lung disease. Programmes include training, health education, breathing techniques, nutritional guidance, and smoking cessation support to name a few.
We bring together healthcare professionals from different fields to collaborate on treatment plans for optimal clinical outcomes in the sub-acute care setting. Primary and allied healthcare practitioners combine their specialities to administer the most comprehensive treatment plans available.
